They have the form on the website to download but it is impossible to fill it in on word document (I think it may be locked or something) ! Is anyone else having this problem? afab Posted November 22, 2015 Share Posted November 22, 2015 I had problem with it too and had to email them. They were very helpful and sent me 2 new versions. In the end it didn't help as the problem seemed to be that I work on a Mac so I opened their form on a PC, filled it in and then transferred it on my Mac and miracle it worked!
